Texas insurance firm calling on beneficiaries of a Fil-Am WWII veteran

The Jackson National Life Insurance Company in Texas has sent a letter to Mr. Richard Ongsotto, Jr. in Carson, California, asking him for some information on the whereabouts of the above-named person, a former housemate of Mr. Ongsotto, concerning a pending insurance claim under his name.
In the final request of information dated Dec. 4, 2013, Ms. Debie Brumley of the company's Claims Services said that the insurance firm has requested items necessary to proceed with its review of the pending claim on the above-named person.
The required items being requested by the company include: a) the enclosed Claimant Statement completed and signed by the named beneficiary. If the beneficiary has had a change in name, the company requires a copy of the applicable marriage license, divorce decree or similar legal documents; b) a copy of the certified death certificate. This should indicated cause of death, manner of death, date of birth and Social Security Number; c) Return the original policy.
If the original policy cannot be located, please note on the Claimant Statement (Page 3, Item 4). Right now, the Claimant Statement form is available with the Mr. Ongsotto in Carson.
According to Mr. Ongsotto, Mr. Pesigan may have died a long time ago in the Philippines. Therefore, he is calling on the immediate relatives of Mr. Pesigan to call or inform the said insurance company so as to check if one of them is qualified to file for the pending unclaimed death proceeds of Mr. Pesigan?
The insurance company is only giving the relatives of Mr. Pesigan in the U.S. or in the Philippines to call the company within 30 days upon receipt of the request for information notice, which is in the possession of Mr. Ongsotto.
